This Sixth Form External Application Form must be completed and submitted by Friday 7th June 2024.
We are legally obliged to collect personal information about parents, carers and students. The Children’s Act 1989 requires us to keep up-to-date details of all people who have parental responsibility for our students.
These include:
1. Parents and carers (whether married or unmarried) who live together.
2. Step-parents or other legal guardians who have obtained parental responsibility either by court order, by appointment in writing by a person with parental responsibility or by a will.
3. The unmarried father (if granted parental responsibility by a court order or by a formal written agreement with the mother).
4. Divorced or separated parents and carers.
5. The Local Authority Social Services Department, which may share parental responsibility with the child’s natural parents by virtue of an order made at court (a care order or similar).
The act also requires that we keep up-to-date details of any court order made under section 8 of the act in respect of our students. These orders include Residence Orders, Contact Orders, Prohibited Steps Orders and Specific Issues Orders. You will only be aware of such an order if it relates to your child.
The act requires that all who have parental responsibility should have equal access to school reports and be involved, if they so wish, in all normal decision-making regarding a student’s education.
We are required to obtain and hold all of the information requested in this form by the Department of Education. You may be assured that all information received will be treated in a confidential manner. Ethnicity
All schools are required by the Department for Children, Schools and Families to collect information on our student's ethnic background. Parents/carers of all students must select one of the options below. Our ethnic background describes how we think of ourselves. This may be based on many things including, for example, our skin colour, language, culture, ancestry or family history. Ethnic background is not the same as nationality or country of birth.
It is recommended that young people over the age of 11 years old have the opportunity to decide their own ethnic identity. Parents/carers are asked to support or advise those children aged over 11 in making this decision, wherever necessary. Students aged 16 or over can make this decision for themselves.
School and Local Activities Consent Form
The education of your child will be greatly benefited if they can participate in the wide range of activities we organise. The range of events includes higher educational visits, geography field work, visits to places of historical interest and many more. We require your consent for local visits for your child whilst they are in Years' 12 and 13.
Such visits would be either on foot, by staff car, minibus, coach or train and would have the appropriate regulation staff supervision and insurance cover. On those occasions when visits involve any kind of cost or voluntary contribution you will receive a letter with details and an attached reply slip as usual.
Although we will make every effort to inform you when your child is involved in such activities, often by email or letter, it is believed that the request for a separate signed parental consent form on each occasion is unnecessary.
It should be emphasised that separate consent forms will still be required for longer trips and residential visits. Students will not be allowed to attend an off-site activity unless we have express parental permission authorising this.
Consent for School Activities
I consent to my child taking part in out of school supervised visits and sporting activities which take place in the local area, in and around Sittingbourne, other areas of Swale, the Medway Towns, Dartford, Maidstone, Canterbury and Ashford.
I have ensured that my child understands that it is important for their safety and for the safety of the group that any rules and instructions given by staff in charge are followed at all times.
I understand that, while the school staff and helpers in charge of the party will take all reasonable care of students, unless they are negligent they cannot be held responsible for any loss, damage or injury suffered by my child during any visits/activities.
I agree to pay for any damages which may be occasioned solely through the misconduct or carelessness of my child to the person or property of any other parties, on the understanding that such agreement should not be interpreted as in any way precluding possible access to insurance claims or other means of redress should such access be deemed more appropriate.
Sixth Form Home-School Contract
This is the final section of our application form and should be completed by the student.
This Contract outlines the terms and conditions under which you will undertake your study during your time in the Sixth Form at Fulston Manor School.
Please read the following carefully, then tick the box at the end of this form to digitally consent to our Home-School Contract.
Expectations and Procedures
• The Sixth Form should not and cannot be viewed as an 8.45am to 3.15pm experience. All students are expected to stay on until at least 4.15pm should a member of staff require it. This is a minimum expectation.
• All students must arrive in school by 8.40am each day and attend tutor at 8.45am.
• Wherever possible you should arrange medical appointments and driving lessons/tests outside of school hours. For medical appointments, please provide an appointment card or letter to our Pastoral Team.
• Students wishing to make changes to their subject choices must discuss their options with all parties concerned, including parents/carers, before final decisions are made by the Head of Sixth Form.
• Lateness, truancy or misuse of independent study time can result in signing out entitlements being withdrawn or compulsory attendance of after school sessions.
• Independent study time is a feature of sixth form life and is built into your timetable. You can make your own suitable arrangements as to where to carry out such work and study, although the Sixth Form Centre is well-equipped for this purpose. All courses work to deadlines and these must be met at all times.
• All sixth form students are also expected to work at home and a minimum of 6–8 hours a week of self-study is essential.
• All sixth form students are expected to set an example to other students in our school in their standard of dress (see Sixth Form Dress Code) and conduct around our school site.
• To ensure that you fully realise your academic potential, you should restrict the amount of paid work that you do outside of school to a minimum. You should avoid working more than 12 hours per week and should NOT work before 4.30pm on any school day.
• Students are only permitted to eat and drink in the Sixth Form Common Room and outside seating area (drinks with lids are permitted in the Study Zone). You have a responsibility to keep the Sixth Form Centre clean and tidy - it is yours to look after.
• Receiving and making phones calls on mobile phones is only permitted in the Sixth Form Common Room. Additional use of mobile phones is permitted as outlined in our Mobile Phone Policy.
• All Year 12 students must arrange and successfully complete a week of work experience during the Summer Term.
Partner Schools
For those completing courses at our partnership schools, you must comply with partner school procedures for signing in and out and leave partner schools once a lesson is completed.
Code of Conduct
If continued or serious complaints regarding the conduct of students are received and further action is deemed necessary, the following procedure will be used:
1) Verbal Warning: To be given by the Head of Sixth Form.
2) Written Warning 1: Warning confirmed in writing, parents/carers will have the opportunity to meet with the Head of Sixth Form.
3) Written Warning 2: Serious or continuing problems will result in Head of Sixth Form informing parents/carers and Executive Headteacher in writing. Parents/carers are invited into school and temporary exclusion is possible.
4) Permanent exclusion: Whilst it is rare for matters to reach this stage, serious breaches of our school rules, or continued breaches could trigger this severe sanction. Appeals are possible.
